Newcastle and Hunter Community Health is seeking compassionate, reliable and experienced Support Workers to join our community care team.
As a Support Worker, you will provide personalised assistance that helps clients maintain their independence, wellbeing and connection with their community.

About the Role
Your responsibilities may include:
- Providing person-centred support to clients in their homes and community
- Assisting with daily living activities
- Supporting personal care needs where required
- Assisting clients with community access and participation
- Providing transport and assistance with appointments or activities
- Supporting clients to work towards their individual goals
- Following individual care and support plans
- Maintaining accurate service records and documentation
- Communicating professionally with clients, families and team members
- Promoting client dignity, choice, independence and wellbeing
- Following workplace health, safety and quality procedures
About You
We are looking for someone who is caring, dependable and comfortable working independently in the community.
You will ideally have:
- Certificate III or IV in Individual Support, Aged Care, Disability, Community Services or a relevant equivalent qualification
- Relevant experience in a support or community care role
- Strong communication and interpersonal skills
- A respectful and person-centred approach
- Good time-management and organisational skills
- Ability to work both independently and collaboratively
- Flexibility to work across a rotating roster where required
Essential Requirements
Successful applicants may be required to provide:
- Australian unrestricted driver’s licence
- Reliable, registered and appropriately insured vehicle
- Current National Police Check or willingness to obtain one
- Current First Aid and CPR certification
- Relevant Working with Children or Working with Vulnerable People clearance where applicable
- Smartphone and internet access
- Relevant professional referees
- Evidence of required qualifications and training
