The individualised support offered to clients has been designed by the team, on their own terms and in a way that meets their goals. This includes people having addition support from their General Practitioner under a GP Management Plan and Team Care Arrangements.

And it’s important to stay socially connected, particularly if living alone. We can transport you to and from sporting and recreational events, take you to the movies, the local shopping centre or cultural centre to help you maintain connections with your social circles.
