NHCH offers Developmental Life Skills Support under our Aged Care services for older people in Newcastle and Hunter.
Developmental life skills may refer to abilities and skills that older adults need to maintain their independence and quality of life as they age. These include important routine skills like self-care, mental health, problem-solving, social interaction, and emotional management. NHCH offers Life Skills Support services tailored to the individual needs and preferences of Consumers.
This service may be covered under your My Aged Care plan package, if you have one. You can contact NHCH to check your eligibility for the same.
The support may cover an assessment of your needs and developing a personalized care plan to meet the specific developmental life skills gaps, assistance at home with daily routine activities to promote independence and skill development, temporary respite care to allow the older person to interact with other people and develop their skills, conducting therapy sessions to improve physical function, mental abilities, and daily living skills, and creating more diverse opportunities for social interaction and engagement to improve social skills.

The Regional Assessment Service will work with you to develop a personalised home support plan tailored directly to your needs. Assessors send a ‘referral for service’ to appropriate providers, such as Newcastle and Hunter Community Health, to let them know you are a new client and would like to engage their services. The provider will contact you to make further arrangements.
Alternatively, if you would like to visit or speak to a service provide to decide if they are the right fit for you, a ‘referral code’ can be sent which allows them to view your client record. The provider will review your identified needs and determine how they can assist. You then make the final decision on whether or not to engage their services.
