Medical Appointment Assistance for Older Citizens

This type of service for older adults may include making a medical appointment on their behalf, providing transport to take them and escorting them when they go to attend the appointment.

A qualified support worker can also assist you with finding the right healthcare provider or doctor for your needs. They can make appointments based on your medical history and help you navigate the healthcare system, ensuring your comfort and safety throughout the process.

The service is usually provided by registered Aged Care agencies like NHCH that have qualified and trained workers to help older people make and attend medical appointments. This is when you do not have a family member or friend who can help with scheduling appointments and escorting you to medical visits.

If you have a Home Care Package, this service may be covered under it. We can help you get assessed if you’re looking to apply for Aged Care support. Contact NHCH today.

What We Do

We at NHCH are committed to bringing convenience to your personal and healthcare routine through tailored care and support, including assistance with medical appointments.

Our comprehensive services include helping older citizens with scheduling and transportation to medical appointments. This service is available mainly for people with age-related restraints, which limit their ability to make or attend medical appointments independently.

Scheduling medical appointments – We provide a dedicated caregiver to help you make a doctor’s appointment, whether you’re seeking an immediate appointment or want to schedule for later. They can explain your condition to the doctor and special needs such as wheelchair access.

Preparing you for medical assessments – The caregiver will prepare you for medical tests. Like, you may have to fast (not eat or drink) for a few hours before a blood test. We’ll also make follow-up appointments, when needed, to collect and discuss the test results.

Taking you to medical appointments (transportation) – Our team can provide transport as well as accompany you to medical appointments. This includes any friendly support you may need.

Managing appointments and medication – We can maintain a proper record of all your scheduled medical appointments, ensuring you do not miss any of them. Our team can also remind you to take your medication on time.

Consumer directed care

The Regional Assessment Service will work with you to develop a personalised home support plan tailored directly to your needs. Assessors send a ‘referral for service’ to appropriate providers, such as Newcastle and Hunter Community Health, to let them know you are a new client and would like to engage their services. The provider will contact you to make further arrangements.

Alternatively, if you would like to visit or speak to a service provide to decide if they are the right fit for you, a ‘referral code’ can be sent which allows them to view your client record. The provider will review your identified needs and determine how they can assist. You then make the final decision on whether or not to engage their services.

If you’re seeking funding under the My Aged Care plan or already have a care package, we can assist you to get the most out of your plan and access affordable care services.
