What does NDIS Post-Hospital Care cover?
The National Disability Insurance Scheme (NDIS) offers funding for post-hospital care to help participants receive optimum care after they leave the hospital following a major surgery or treatment. This care may include support workers to assist with daily tasks, transport, etc. and trained nurses to administer ongoing care, medication, pain management, and more.
At NHCH, we offer comprehensive Post Hospital Care services focused on your comfort, well-being and speedy recovery after surgery. We understand that each individual has specific care needs and so we offer tailored services that are personalised to your specific requirements and preferences.
To start with, our team will create a Personalised Care Plan tailored to you. We work closely with your care providers and family to understand your goals and create personalised care plans based on your medical condition and lifestyle. Our caregivers and nurses are highly trained and provide skilled care with compassion to individuals recovering from surgery or serious medical conditions.
We prioritise your safety and comfort at all times while delivering services. Our nurses can also assist with ongoing monitoring and medication management, including administering prescribed medications on time and regularly monitoring and recording your physical condition, vital signs, and overall recovery progress. They can handle emergencies as well.

The Regional Assessment Service will work with you to develop a personalised home support plan tailored directly to your needs. Assessors send a ‘referral for service’ to appropriate providers, such as Newcastle and Hunter Community Health, to let them know you are a new client and would like to engage their services. The provider will contact you to make further arrangements.
Alternatively, if you would like to visit or speak to a service provide to decide if they are the right fit for you, a ‘referral code’ can be sent which allows them to view your client record. The provider will review your identified needs and determine how they can assist. You then make the final decision on whether or not to engage their services.
If you’re looking for someone to assist with Post Hospital Care or want to apply for the NDIS plan, contact us to talk to an expert about the best recovery plan for you.
