When you reach a stage in your life where a little extra assistance could make meeting your daily needs easier, our team of professional health care workers offer a friendly, helping hand and can support you to create a customised Health Care Package to suit your requirements.

What is Consumer Directed Care (CDC)?

Consumer Directed Care is a model of service delivery specifically designed to ensure that patients have more control over their health care.

The CDC model allows for more flexibility with supported care packages, ensuring you have the final say over the type of service you get, who delivers it and where you receive it.

As an authorised government provider, NHCH will be a partner in your care and provide transparency in all our dealings.

How does it work?

Consumer Directed Care allows for you to have full control over where your funding is being allocated within your Home Care Package.

Through NHCH, there are four different levels of home care packages available to our patients, which include;

  1. Support for people with basic care needs;
  2. Support for people with low level care needs;
  3. Support for people with intermediate care needs; and
  4. Support for people with high level care needs.

Finding the right care starts with a conversation. We’ll talk to you about your goals, beliefs, religious and lifestyle choices, and the level of support you need so we can provide the right level of health care service for you.

What services are available through NHCH?

We understand the importance of tailoring your health care to suit your home and personal needs, because we know that everyone is different. We offer a variety of services for you to choose from to suit your needs.

These include but are not limited to:

  • Assistance with transport, shopping, and other everyday tasks;
  • Domestic services;
  • House and yard services;
  • Social and community access services e.g. maintaining social contact with family and friends;
  • Personal care; and
  • Clinical care, including assistance with wounds, medication and chronic disease management.

Our team will take you through these services so you understand how they can benefit you and which are the best fit for your goals and requirements.
