Our number one goal at Newcastle and Hunter Community Health is to provide the support needed for our clients to achieve independent living.

As a locally based, family owned and operated business, we understand how important the health and wellbeing of our loved ones is to their quality of life. With the support of our personalised care planning, we work together with our clients and their carers to ensure they have the support needed to live their best life.  

We are an approved National Disability Insurance Scheme (NDIS) and Aged Care Provider staffed by dedicated Registered Nurses, Enrolled Nurses, Assistants in Nursing and professional cleaners who deliver tailored medical, clinical and personal care, nursing and allied health services, community support and domestic services. 

While we are providing in-home services to meet your clinical and life goals, we also know it’s sometimes a particular task which can be challenging to get assistance with. Our individualised assessment ensures we know about all your important independent living activities such as assistance with sensory communication aids, medication management, diabetes management, hygiene and personal care, wound care and the like, to make daily life easier.    

Individual and group community participation is vital to remaining connected to others, so we make it a priority to offer supported shopping and opportunities to attend sporting and recreational events and transport to enable participation. 

NHCH can also help with life stage transitions and skill development, including with accommodation and tenancy obligations, as well as budgeting, peer support, mentoring and decision making to support people make transitions on their own terms and in a way that meets their goals and aspirations. 

Importantly, we also offer carer support. Sometimes, people forget that carers have individual needs too so whether it’s short-term, long term or a regular planned activity with the NDIS or Aged Care recipient, we always make respite for carers an integral part of our care plans.  

Whether home care is self-funded, or government provided, or a combination of both, NHCH can assist with almost anything to make our clients feel safe, secure and independent in their home and continue to live a happy and fulfilled life doing the things they enjoy. 

Our business is about knowing our clients. We are small enough to know each individual, yet big enough to provide optimum care to those clients using our services.
