Newcastle and Hunter Community Health is an approved Department of Veterans’ Affairs (DVA) Community Nursing Services provider.
Helping those who have represented our country
If you or your loved one has represented Australia in active military service, you may be eligible to receive personalised DVA Community Nursing Services.
DVA Community Nursing Services are available to provide practical help to support your continued independence and ensure you get the care you need to remain living at home for as long as practical.
Administered through the Department of Veterans’ Affairs, the program is offered to eligible carriers of a Veteran Gold Card or a Veteran White Card.
What’s provided under the DVA Community Nursing Services?
The team at Newcastle and Hunter Community Health is pleased to provide the following DVA Community Nursing Services:
- community nursing care for wounds, skin integrity
- assessment and monitoring
- medication assistance
- personal care assistance
Clients need a referral to DVA Community Nursing Services if they are new to receiving the services or are starting a new episode of care.
Who can refer clients
To provide DVA Community Nursing Services to a client, the referral must come from an authorised referral source. These include:
- a General Practitioner (GP)
- a Nurse Practitioner specialising in a community nursing field
- a treating doctor in hospital
- a hospital discharge planner.
Upon receiving a referral from an eligible referrer, Newcastle and Hunter Community Health will schedule your assessment and book in your clinical need services.
